I agree to permit the review of this application by members of the Fayette Electric Cooperative Scholarship Committee.

If you are chosen as a scholarship recipient, your parent/guardian will be required to fill out an affidavit certifying that you do live with them in a residence served by this cooperative. They will also be asked to sign a photo release form allowing your photo to be used in various publications, on social media posts announcing the winners and on Fayette Electric Cooperative's website.
